dc.description.abstractA Piroplasmose Equina (PE) e Anaplasmose Granulocítica Equina (AGE) são enfermidades transmitidas por carrapatos ixodídeos, que acometem equinos em várias regiões do mundo. São causadas pelos protozoários Babesia caballi, Theileria equi e Theileria haneyi e pela bactéria Anaplasma phagocytophilum, respectivamente. As duas enfermidades podem se manifestar como condições subclínicas ou agudas, provocando doença febril inespecífica, o que dificulta a diferenciação clínica. No entanto, as abordagens terapêuticas são distintas. Estudos sorológicos e moleculares realizados em cavalos no Brasil evidenciaram a presença de A.phagocytophilum, mas a identificação desse agente em equinos com manifestações clínicas no país ainda são poucas, o que coloca em dúvida a sua relevância clínica. Desta forma, objetivou-se com o presente trabalho avaliar a importância de A. phagocytophilum, B. caballi e T. equi, como agentes etiológicos em equinos com manifestações clínicas sugestivas de PE, além de determinar a proporção de pacientes infectados com cada agente e descrever clínico e laboratorialmente os achados obtidos de acordo com o hemopatógeno identificado e, com isso, dar subsídios aos clínicos veterinários para a realização do diagnóstico, que orientará a escolha terapêutica. Amostras de sangue da veia jugular de animais que apresentassem pelo menos três sinais clínicos de PE foram submetidas a análises hematológicas e moleculares (qPCR). Dos 45 animais avaliados com sinais clínicos, em nenhum deles (0%) ocorreu detecção de A. phagocytophilum, em cinco (11,2%) foi detectado apenas de B. caballi, em 16 (35,5%) apenas de T. equi, sete (15,5%) demonstraram a presença dos dois hemoprotozoários e em 17 (37,8%) não houve detecção de DNA de nenhum dos agentes pesquisados. A análise estatística dos dados demonstrou que a PE é significativamente mais importante do que a AGE. Entre os casos de PE, T. equi foi o agente etiológico associado a maioria dos casos clínicos. Outras etiologias também podem causar síndromes com sinais inespecíficos em cavalos. Os sinais clínicos e os parâmetros hematológicos tendem a ser semelhantes nas doenças febris inespecíficas. Edema de membros demonstrou ser mais comum nos casos de outra etiologia do que nas infecções por T. equi e coinfecções por Piroplasmidas. Hiporexia é mais frequentemente observada nos casos de B. caballi do que nas coinfecções por Piroplasmidas e letargia está mais presente nos animais coinfectados pelos dois protozoários do que em outras causas. VCM e HCM também demonstraram tendência a um valor inferior nas coinfecções por Piroplasmidas quando comparada a outras causas. Por fim, AGE não deve ser considerada como uma suspeita principal nos casos de síndrome febril inespecífica na Mesorregião Metropolitana do estado do Rio de Janeiro. Protozoários causadores de PE em infecções simples ou múltiplas são frequentemente encontrados em cavalos com manifestações clínicas, representando um importante diagnóstico diferencial nos casos suspeitos nessa região. Os clínicos veterinários devem se atentar não só aos sinais clínicos e exames hematológicos, mas também ao histórico do animal e à epidemiologia da região para levantar a suspeita diagnóstica. Exames moleculares possibilitam a determinação de um diagnóstico definitivo e devem ser usados sempre que possível, guiando a escolha terapêutica adequada para cada caso.pt_BR

dc.description.abstractOtherEquine Piroplasmosis (EP) and Equine Granulocytic Anaplasmosis (EGA) are diseases transmitted by ixodid ticks, which affect horses in various regions of the world. They are caused by the protozoa Babesia caballi, Theileria equi and Theileria haneyi and by the bacterium Anaplasma phagocytophilum, respectively. Both diseases can manifest as subclinical or acute conditions, causing nonspecific febrile illness, which makes clinical differentiation difficult. However, the therapeutic approaches are different. Serological and molecular studies carried out in horses in Brazil have shown the presence of A. phagocytophilum, but the identification of this agent in horses with clinical manifestations in the country are still few, which puts its clinical relevance in doubt. Thus, the aim of this study was to evaluate the importance of A.phagocytophilum, B. caballi and T. equi as etiological agents in horses with clinical manifestations suggestive of EP, in addition to determining the proportion of patients infected with each agent and to describe the clinical and laboratorial findings obtained according to the identified hemopathogen and, therefore, provide subsidies to veterinary clinicians to carry out the diagnosis, which will guide the therapeutic choice. Blood samples from the jugular vein of animals that presented at least three clinical signs of EP were submitted to hematological and molecular analyzes (qPCR). Of the 45 animals evaluated with clinical signs, none of them (0%) detected A. phagocytophilum, in five (11.2%) only B. caballi was detected, in 16 (35.5%) only T. equi, seven (15.5%) demonstrated the presence of both hemoprotozoa and in 17 (37.8%) there was no detection of DNA from any of the investigated agents. Statistical analysis of the data showed that EP is significantly more important than EGA. Among EP cases, T. equi was the etiologic agent associated with most clinical cases. Other etiologies can also cause syndromes with nonspecific signs in horses. Clinical signs and haematological parameters tend to be similar in non-specific febrile illnesses. Limb edema has been shown to be more common in cases of other etiology than in T. equi infections and Piroplasmid co-infections. Hyporexia is more frequently observed in cases of B. caballi than in Piroplasmid co-infections, and lethargy is more present in animals co-infected with both protozoa than in other causes. VCM and HCM also tended towards a lower value in Piroplasmid co-infections when compared to other causes. Finally, EGA should not be considered as a prime suspect in cases of nonspecific febrile syndrome in the metropolitan mesoregion of the state of Rio de Janeiro. Protozoa that cause EP in single or multiple infections are often found in horses with clinical manifestations, representing an important differential diagnosis in suspected cases in this region. Veterinarians should pay attention not only to clinical signs and hematological exams, but also to the animal's history and the epidemiology of the region to raise the diagnostic suspicion. Molecular exams make it possible to determine a definitive diagnosis and should be used whenever possible, guiding the appropriate therapeutic choice for each case.en
