| dc.description.abstract | A sepse é uma síndrome de disfunções orgânicas frente a uma resposta desregulada do organismo a uma infecção Certamente a disfunção cardiovascular se destaca em incidência e influência na morbidade do paciente desta forma, esse estudo clínico teve a proposição de avaliar diferentes protocolos de fluidoterapia associados à anestesia inalatória ou total intravenosa, na estabilidade hemodinâmica de cadelas com sepse, submetida a procedimento cirúrgico, para controle do foco infeccioso. Foram incluídas 32 cadelas, com diagnóstico de piometra e sepse (foco infeccioso mais uma disfunção orgânica), atendidas no HVPA – UFRRJ no período de 2018 a 2019. Após admissão, diagnóstico, avaliação clínica e laboratoriais, os pacientes foram distribuídos de maneira aleatória nos grupos Propofol 5 (P[5] - fluidoterapia restritiva pré-operatória na taxa de 5mL/kg/h e anestesia geral intravenosa), Propofol 10 (P[10]- fluidoterapia liberal pré-operatória na taxa de 10mL/kg/h e anestesia geral intravenosa) e Isofluorano 5 (I[5] – fluidoterapia restritiva pré-operatória na taxa de 5mL/kg/h e anestesia geral inalatória). Foram analisados os seguintes parâmetros: Lactato na admissão (LAC 1), pré cirúrgico (LAC 2) e na liberação (LAC 3) freqüência cardíaca (FC), freqüência respiratória (f), pressão arterial sistólica (PAS),temperatura retal (TEMP), saturação de oxigenio (SpO2) e taxa de extração de dióxido de carbono (ETCO2) nos seguintes momentos PRE, T10, T20, T30, T40, T50, TEXT e TLIB.A depuração de lactato ( LAC 3-LAC 1) ] foi positivo somente no grupo P[5], com o grupo P[10] tendo os melhores valores. Não houve diferença estatística no requerimento de vasopressores entre os grupos. A fluidoterapia liberal apresentou maior estabilidade cardiovascular quando comparado com a fluidoterapia restritiva no período perioperatório. Com relação a anestesia geral, o isoflurano apresentou maior estabilidade cardiorespiratória que o propofol na manutenção anestésica. Os resultados demonstraram que, apesar de todos os três protocolos propostos serem seguros e sem diferença em superioridade entre eles, algumas alterações observadas podem ser relevantes e ponderadas quando for possível a individualização da terapêutica no paciente. | pt_BR |

| dc.description.abstractOther | Sepsis is a syndrome of organic dysfunctions due to an unregulated response of the organism to an infection Certainly cardiovascular dysfunction stands out in incidence and influence on the patient's morbidity. Thus, this clinical study had the purpose of evaluating different fluid therapy protocols associated with anesthesia. inhalation or total intravenous, in the hemodynamic stability of bitches with sepsis, submitted to surgical procedure, to control the infectious focus. 32 bitches were included, diagnosed with pyometra and sepsis (infectious focus plus an organic dysfunction), attended at HVPA - UFRRJ in the period from 2018 to 2019. After admission, diagnosis, clinical and laboratory evaluation, patients were randomly assigned to groups Propofol 5 (P [5] - preoperative restrictive fluid therapy at a rate of 5mL / kg / h and intravenous general anesthesia), Propofol 10 (P [10] - preoperative liberal fluid therapy at a rate of 10mL / kg / h and general anesthesia intravenous) and Isofluorane 5 (I [5] - preoperative restrictive fluid therapy at a rate of 5mL / kg / h and general inhalation anesthesia). The following parameters were analyzed: Lactate at admission (LAC 1), pre- surgical (LAC 2) and at release (LAC 3) heart rate (HR), respiratory rate (f), systolic blood pressure (SBP), rectal temperature (TEMP ), oxygen saturation (SpO2) and carbon dioxide extraction rate (ETCO2) at the following moments PRE, T10, T20, T30, T40, T50, TEXT and TLIB. Lactate clearance (LAC 3-LAC 1)] it was positive only in group P [5], with group P [10] having the best values. There was no statistical difference in the requirement for vasopressors between the groups. Liberal fluid therapy showed greater cardiovascular stability when compared to restrictive fluid therapy in the perioperative period. Regarding general anesthesia, isoflurane showed greater cardiorespiratory stability than propofol in anesthetic maintenance. The results showed that, although all three protocols proposed are safe and with no difference in superiority between them, some observed changes can be relevant and considered when it is possible to individualize therapy in the patient. | en |
